Help for: Update Registration

Use this form to notify MHRA if your registration details change. This may be just a change of address or telephone number, or it may be a change of your password. If you are reporting as part of a haemovigilance team and are using a shared SABRE Registration, please ensure that team members are made aware of password changes.

NOTE: If you change your email address or password, your account will be temporarily suspended pending re-verification by MHRA. This is a routine security measure designed to ensure that all such changes are verified prior to implementation.

If you have any problems e.g. you require a change to be made urgently, please contact the Adverse Incident Centre (AIC) on 020 3080 7336 and be prepared to provide your account number and to answer other questions in order to confirm your identity. AIC staff will do their best to effect any urgent changes quickly.
